Diabetes Eye Exam

What is this exam?
A dilated eye exam for people who have diabetes that checks the health of your eyes and vision.
Who can earn this reward?
You must have a diabetes eye exam between Jan. 1, 2024 and Dec. 31, 2024. Request your reward by Jan. 31, 2025. You will need to provide the date of visit, doctor’s name (first and last) and the name of clinic or location.

Colon Cancer Screening
Get either a $100 (colonoscopy) or $15 (fecal occult blood test or fecal immunochemical test) gift card to CVS, Walmart or Best Buy when you complete your colon cancer screening.

What is this exam?
There are a few types of screenings. A fecal occult blood test and fecal immunochemical test check your stool for hidden blood. A colonoscopy lets your doctor look at your colon for anything abnormal.
Who can earn this reward?
Advantage MD members who are eligible for a colorectal screening exam.* One reward per member in 2024.
How can I earn this reward?
You must have a colorectal screening between Jan. 1, 2024 and Dec. 31, 2024. Request your reward by Jan. 31, 2025. You will need to provide the date of visit and the type of test.

Women’s Bone Density Scan
Get a $100 gift card to CVS, Walmart or Best Buy when you complete your bone density scan.

What is this exam?
A bone density test uses X-rays to measure the amount of calcium and other bone minerals in a portion of your bones. The results can determine if your bones are fragile and more likely to break.
Who can earn this reward?
Advantage MD members ages 67 to 85 who are at risk for developing osteoporosis and have suffered a fracture. Bone density scan must be completed within six months of having the fracture.* One reward per member in 2024.
How can I earn this reward?
You must have a bone density scan between Jan. 1, 2024 and Dec. 31, 2024. Request your reward by Jan. 31, 2025. You will need to provide the date of visit, doctor’s name (first and last) and the name of clinic or location.

What is this exam?
A blood test taken by a finger prick that measures your blood sugar levels over the last few months. The results are used to determine whether you have diabetes or prediabetes or to measure how well you are controlling your blood sugar.
Who can earn this reward?
Advantage MD members who are eligible for an HbA1c test. One reward per member in 2024.
