Sometimes, managing all of your health care needs can feel like too much.

Care managers are nurses and social workers who can help you better understand and manage your health conditions. They are advocates, who can coordinate your care with your providers to make it easier for you to achieve your best health.

Your care manager and health care providers work together with you to:
  • Assess your wellness and personal needs
  • Coordinate health care services and supplies
  • Build your skills so you can better manage your conditions
  • Assist with referrals to specialty providers
You should work with a care manager if you:
  • Are being discharged from an inpatient stay
  • Have a chronic health condition such as diabetes, COPD, hypertension, etc.
  • Want assistance with advance care planning

Preventive Health

We remind you of the services you need for better health and provide information you can trust. We may also arrange a care manager, who can teach you about a new diagnosis, with all of your providers and help you get the most out of your plan.

Transition of Care

Care Managers help navigate post-ER visits, hospital stays, or life changes by assisting with follow-up care, medication access and transitions, reducing hospital visits. Advantage MD (HMO) members also get nutritious meal delivery for five days after discharge.

Complex Care

If you have chronic conditions, we work with you and your providers to help you understand and take better care of your complex health conditions. We help coordinate your care and work one on one to make it easier for you to focus on your health.

Mental Health Support

Taking care of your mental health is as important as taking care of your
physical health.

We can help you with care coordination for substance abuse challenges or struggles with depression, anxiety, or other mental health conditions.
