Advantage MD members get ongoing support when and where it matters most:

24/7 Nurse Chat

Speak directly to a registered nurse 24 hours a day, 7 days a week. Use this service if you have a question and your provider is unavailable. Members use Nurse Chat to ask about symptoms, determine if they need care, and more.

Transition of Care

Care Managers help navigate post-ER visits, hospital stays, or life changes by assisting with follow-up care, medication access, and transitions, reducing hospital visits. Advantage MD (HMO) members also get nutritious meal delivery for five days post-discharge.

Care Management

Our goal is to keep you healthy and help manage your condition. Care managers provide education, one-on-one support, assess needs, collaborate with providers, and connect you to resources. If you need help, we’re here.

Mental Health Support

Taking care of your mental health is as important as taking care of your physical health.

We can help you with care coordination for substance abuse challenges, or struggles with depression, anxiety, or other mental health conditions.

Health & Wellness Classes

Advantage MD members can enjoy free virtual health education workshops and programs to help you achieve your best health. Classes range in topics, here are just a few examples:

Diabetes: A Healthier You

This 60-minute virtual diabetes workshop focuses on the importance of nutrition and physical fitness for adults to live healthier with diabetes. Members will learn how to count carbs, basic meal planning, and how to aim for a healthy weight. The impact of physical activity on diabetes, and what to do, will also be discussed.

Blood, Why are You Pressuring Me?

This 60-minute virtual workshop focuses on learning the warning signs of uncontrolled blood pressure and its effects on your health. Participants learn ways to manage their blood pressure by eating healthy, lowering sodium intake and being physically active.

Eating the Healthy Way

This 60-minute workshop is aimed to provide participants with the tools necessary to make healthy choices throughout the day. Ways to incorporate physical activity into a busy day will also be discussed, along with specific ideas for all 3 main meals. Participants also learn practical ways to shop and plan ahead while staying within a budget.

The Silver&Fit® Exercise and Healthy Aging Program

Workouts. Classes. Newsletters. Rewards. The Silver&Fit® program—designed specifically for older adults—gives members the option to workout at any participating fitness center near them and order home fitness kits to stay in shape at home.

The Johns Hopkins online Health Library connects you to multimedia resources, a health glossary, interactive tools, and more to help you take control of your health.

Personal Care Plan

Could you use a care manager to help you better manage a condition? Perhaps you need preventive services — we are here to help. As a member of our plan, we’re asking you to complete a personal care plan.

A personal care plan is a short questionnaire about your health and lifestyle that gives a profile of your overall health and well-being. Your honest answers to these questions tells us how we can best provide the resources and services you need to support your individual health.

To complete your personal care plan, enter your Advantage MD member ID number and the password “mywellness”.

Completing your personal care plan will help you:

  • Identify existing behaviors that are considered good health practices.
  • Identify protective and health risk factors.
  • Get a comprehensive picture of your health and lifestyle.
  • Make healthy life decisions.
