If you choose to provide us with personal information by sending an email, or by filling out a form with your personal information and submitting it through our Web site, we use that information to respond to your message and to help us provide you with information or material that you request. We do not give, share, sell, or transfer any personal information to a third party unless required by law.
Sometimes, managing all of your health care needs may be too much for you. When that happens, ask for a care manager. These nurses and social workers can help you better understand and manage your health conditions. They can help coordinate your care with your providers. They can make it easier for you to achieve your best health.
Managers can provide you guidance on timely health screenings, annual well checks, immunizations, as well as refer you to our health education classes and health library to help you manage your health and stay as healthy as possible. You may also be able to work with a care manager, who can work with your providers, help you understand a new diagnosis, and use your plan.
Care Managers can provide you with assistance navigating the health care system following a health event such as an emergency room (ER) visit, hospitalization, or significant life event. Care Managers can help you get the appropriate provider follow-up after an ER visit, hospitalization or significant life event, as well as access to the medications or other medical supplies you need to continue your treatment. Care Manager support as you transition from one treatment setting can help you better manage your health and reduce the need to go to the hospital.
Partner with a licensed nurse or social worker care manager to help reach your best level of health. Learn more about a new diagnosis or improve your knowledge and skills in managing a chronic health condition. We work with you and your providers to help you understand and take better care of your complex health conditions. We help coordinate your care and work one-on-one to make it easier for you to focus on your health.
Receive confidential support from a licensed clinical social worker if you have a mental health or substance abuse concern. A behavioral health clinician can help you:
Health Educators advocate, encourage and teach about healthy lifestyles and living well with a chronic condition. Attend one of our many interactive programs to give you information, tips and tools to better manage your health.
