Dual Special Needs Plan (D-SNP)
If you’re already enrolled in both Medicare and Medicaid and reside in one of our covered counties, you may qualify to enroll in a Dual Special Needs Plan (D-SNP).
A D-SNP is designed for individuals who are eligible for both Medicare and Medicaid. These plans provide the same Medicare benefits as other Medicare Advantage plans, with the added potential for additional services and benefits.
Plan Overview:
- $0 monthly premium
- $0 for most health care services
- No deductibles
- Prescription drug coverage
Plan Benefits
Dental
Preventive and comprehensive dental services are included in the D-SNP plan for a $0 copay.
Hearing
We cover routine hearing exams and $1,500 toward the purchase of hearing aids every 24 months.
Vision
D-SNP members pay nothing for routine and diagnostic eye exams, plus get a $400 allowance every year for eyewear.

The Dual Special Needs Plan from Advantage MD features all the benefits of Medicaid and Medicare, plus extra benefits only available through Advantage MD. This all-in-one plan can make your health care easier to manage at little to no cost to you.

- Dental, vision and hearing benefits
- $100 allowance for over-the-counter products, every three months
- Transportation to plan-covered services
- $0 home fitness or gym memberships
Personal Care Plan for Advantage MD HMO (D-SNP) Members
Could you use a care manager to help you better manage a condition? Perhaps you need preventive services — we are here to help. As a member of our plan, we’re asking you to complete a personal health risk assessment (HRA).
An HRA is a short questionnaire about your health and lifestyle that gives a profile of your overall health and well-being. Your honest answers to these questions tells us how we can best provide the resources and services you need to support your individual health.
To complete your HRA, enter your Advantage MD member ID number and the password “mywellness”.
Completing your personal HRA will help you:
- Identify existing behaviors that are considered good health practices.
- Identify protective and health risk factors.
- Get a comprehensive picture of your health and lifestyle.
- Make healthy life decisions.
Flex Card: Extra $90 Monthly Allowance
With the D-SNP plan, you could receive a Flex Card* to help cover some of your expenses. Each month, D-SNP members receive $90 on their Flex Card.
You can use your allowance to help pay for healthy food and utilities. The card arrives pre-loaded, and we will refresh it with a new $90 every month — all you have to do is spend it.
*This benefit is part of a special supplemental program for the chronically ill. Not all members qualify.
