In-Home Health Assessments

Perk: no-cost visits with a licensed clinician from the comfort of home
It’s ok, stay home. With this perk from Advantage MD, a licensed clinician will come to you. During this no-cost visit, members will spend up to one-hour with a clinician to have their medication and medical history reviewed, have their vital signs checked and ask questions. This is an extra visit beyond your wellness visit. Members eligible for this visit may also receive a $25 gift card. Virtual visits are also available. If eligible, Advantage MD or a representative from Signify Health will contact you.

 

My Advocate

Perk: Personalized assistance to find and apply for programs to save you money
Advantage MD is collaborating with the My Advocate program from Change Healthcare to help you save on your health care expenses. If you qualify, a program advocate will help you find and apply for government programs that could help you pay less for your health plan premiums and prescription drugs. Advantage MD or a representative from My Advocate may contact eligible members to get started.
