This all-in-one plan can make your health care easier to manage at little to no cost to you.

Flex Card: Extra $90 Monthly Allowance
With the D-SNP plan, you could receive a Flex Card* to help cover some of your expenses. Each month, D-SNP members receive $90 on their Flex Card. You can use your allowance to help pay for healthy food and utilities. The card arrives pre-loaded, and we will refresh it with a new $90 every month — all you have to do is spend it.
*This benefit is part of a special supplemental program for the chronically ill. Not all members qualify.
Dental
Preventive and comprehensive dental services are included in the D-SNP plan for a $0 copay.
Hearing
We cover routine hearing exams and $1,500 toward the purchase of hearing aids every 24 months.
Vision
D-SNP members pay nothing for routine and diagnostic eye exams, plus get a $400 allowance every year for eyewear.
